Massage Therapy Terms and Conditions:

Welcome to Starlight Haven massage therapy services. Before we proceed with any massage therapy sessions, we require all clients to read, understand, and agree to the following terms and conditions. By checking the box, you acknowledge that you have read, understood, and agreed to comply with these terms and conditions.

Consent to Treatment

1. Voluntary Participation: I understand that my participation in massage therapy sessions is entirely voluntary and that I am free to withdraw my consent and discontinue participation at any time.
2. Health History: I agree to disclose my health history accurately and fully, including any physical or medical conditions, allergies, and medications, to my massage therapist before the session begins. I understand that withholding information may result in adverse effects or injury during or after my massage therapy session.
3. Therapeutic Purpose: I understand that massage therapy is designed to be a health aid and is not a substitute for medical treatment or diagnosis. I acknowledge that the massage therapist does not diagnose illnesses or injuries, prescribe medications, or offer medical advice.

Acknowledgment of Risks

1. Potential Risks: I understand that massage therapy involves physical touch and manipulation of muscles and soft tissues. While rare, there are risks of injury, discomfort, or other complications. I assume full responsibility for any risks, injuries, or damages, known or unknown, arising from massage therapy sessions.
2. Reporting Discomfort: I agree to immediately inform my massage therapist of any discomfort or pain during the session, as well as any concerns I have about the therapy's nature and execution.

Acknowledgment and Agreement

I have read and understood the terms and conditions outlined in this waiver. I agree to abide by these terms and understand that failure to adhere to these guidelines may result in the termination of massage therapy services.
